Per my acceptance to participate in the 9th Annual Friends of the National World War II Memorial Teachers Conference in Washington, DC (Please check next to each commitment):

Travel

Friends will provide housing: each conference participant will be randomly assigned to a double-occupancy room with a fellow teacher of the same gender. Roommate requests and preferences will be accommodated within reason and space availability. Potential for mixed gender within one double-occupancy room can be considered upon request. Please Note: Each participant will be required to provide the Hyatt Centric Arlington with a credit card upon check-in to cover any incidentals (beyond room and tax).

Friends will award a travel stipend: attendees within a 250-mile radius of Washington, DC will receive a travel stipend up to $150 based on the 2024 federal mileage rates. Participants outside of the 250-mile radius of Washington, DC will receive a travel stipend up to $400. Participants will be expected to provide Friends with travel expense receipts to receive stipend.

• Friends will cover parking expenses for participants who choose to commute.
• Friends will provide breakfast and lunch daily.

Remembrance Ceremony Honoree (optional)

On Friday, July 19, 2024, at approximately 10:15 A.M., members of the Friends of the National WWII Memorial Teachers Network and Conference will participate in a Remembrance Ceremony at the National WWII Memorial.  The purpose of the ceremony is to remember and honor the 16 million who served in the U.S. armed forces during WWII, the more than 400,000 who died, and all who supported the war effort from home.

As part of this ceremony, we’re inviting you to provide brief remarks about a family member, friend, or member of your community who served during WWII – at home or abroad. Please indicate in the space provided, the name of the person you would like to honor.  Please provide as much information as possible.

If you would like us to profile your WWII Veteran / Member of the Greatest Generation on the Friends’ Facebook page, please provide a bio and picture(s) when you submit this form.

Responsibility Acknowledgement and Liability Disclaimer

In furtherance of its mission to honor and preserve the national memory of World War II, the Friends of the National World War II Memorial (“Friends”) organizes educational programs and conference (“Events”). This agreement is between the undersigned (“Participant”) and Friends concerning the terms and conditions of participating in such Events.

Friends organizes Events featuring travel to and tours of facilities that are maintained and operated by third parties not under the control of Friends. In addition, Friends arranges for travel, accommodations, and other services through third party suppliers not under the control of Friends. During an Event, Participant is responsible for abiding by all policies, regulations and requirements of Friends and third parties, including policies for the preservation of historic artifacts and locations. Participant’s participation in an Event may be cancelled or suspended by Friends for any period of time in the event any conduct by the Participant, in the sole opinion of Friends, is prejudicial to Friends or an Event.

Events may include activities requiring physical agility and mobility. Participant acknowledges and agrees that it is the Participant’s responsibility to investigate and understand the nature of activities during an Event and to determine that he/she is in appropriate physical condition to participate in the Event.

Friends reserves the right to make changes in the planned schedule whenever, in its judgment, conditions warrant, or if it deems it necessary for the comfort, convenience, or safety of participants. Friends also reserves the right to cancel any Event, to decline to accept any person as a Participant in an Event, or to require any participant to withdraw from an Event at any time, when such action is determined by Event staff to be in the best interests of the health, safety or general welfare of the Event group or of the individual Participant.

In addition, there are many risks and uncertainties inherent in any Event, including but not limited to the hazards of various modes of transportation, forces of nature, terrorism, war or insurrection, theft, illness, and damage to person or property due to the negligent acts or omissions of Event staff or others. Friends shall not be held liable for personal injury, property damage, theft or damage of any kind, including but not limited to economic loss, suffered by the Participant in connection with any Event, whether due to the negligence of Event staff, third parties, or others.

The Participant has carefully read the Responsibility Acknowledgement and Liability Disclaimer set forth above and the schedule of activities for this Event and recognizes and accepts any risk associated with the Event. In consideration of the right to participate in the Event, the Participant, on behalf of him/herself, his/her dependents, heirs, executors, administrators and assigns, agrees to release Friends, its Board members, officers, employees, representatives or agents, and other participants from any and all liability for personal injury, property damage, theft or damage of any kind, including but not limited to economic loss, suffered by any person in connection with the Event.

In addition, by registering for the Event, the Participant certifies that he or she is mentally and physically capable of full participation in the Event.
